This downloadable collection includes four PEAR TREE™ About Me versions designed for School, Work, Community and Hospital settings. Each version offers a simple, structured way to record personalised information under four connected areas: Person, Environment, Activity and Relational Response.
The resource recognises that a person’s experience and participation may change depending on where they are, what they are being asked to do and how others communicate with and respond to them. It provides space to capture what matters to the individual, what others may need to understand, what may make participation easier or more difficult and what respectful support could look like within each setting.
The resource can be completed independently or collaboratively with families, educators, employers, carers or healthcare professionals. Once completed, it can be used as a clear reference to support communication, shared understanding and continuity across different people and services.
